Completed registration forms can now be returned to the surgery via email - please send your completed GMS1 form and Health Questionnaire, along with copies of photo ID and proof of address, to firstname.lastname@example.org. If you do not have ID or proof of address, please telephone the surgery and speak to a receptionist. Please ensure your registration forms are completed correctly. If you were born outside of the UK, we MUST have your date of coming into the country to process your registration. Your registration should be complete within 3 week and you will receive a text message to confirm this.
We understand that not all appointments can be booked in advance and that some patients experience medical problems that need more urgent attention. We have a telephone triage system each day - if you require urgent advice, please call before 10am and a message will be left with the duty doctor. You will be called back on the same day with further advice. You may not be offered an appointment on the day if the duty doctor does not think this is necessary.
You will be asked by the receptionist the reason for the urgency; this is under the instruction of the Partners of the Practice and you must give an answer as to why they need to phone you back.
Appointments can be pre-booked up to 4 weeks in advance via our online booking service or by calling the surgery.
Please only phone in the afternoon for test results. All results are checked by the GPs before the Receptionists are able to give you the results. Should a result need you to come and see a Clinician, you may be telephoned by a Receptionist to book an appointment.
New Patient Health Check
As a new patient you may be required to have a new patient health check if you are aged 40 or over and do not already have a diagnosis of high blood pressure, heart disease, diabetes or kidney disease. This will comprise of a general check up; normally including a urine sample to test for underlying medical issues like diabetes.
If you are on a repeat medication, you can request your medication online (if you have signed up for this). You can drop a repeat medication request through the letter box if we are closed, or put in your request direct to reception.
If you are due a medication review, you will need a non urgent appointment with a GP. You cannot order your repeat medication online if your review is over due.
